Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to falls from elevation:
On the south end of building #2, an employee who was erecting a pump jack
scaffold
was elevated in a personnel platform on the forks of a rough terrain
forklift.  While in
the elevated platform, the employee did not maintain firm footing on the
floor of the
platform and was not secured inside the platform by a fall restraint
system.  The
employee climbed and stood on the mid rail of the basket's guardrail
system in order to
achieve additional reach or height, exposing the employee to the hazard of
a fall from
the personnel platform.
Among other means, acceptable and feasible means of abatement include:
1.  Provide employees working in elevated personnel platforms with a fall
restraint system (i.e. body belt or harness and lanyard) to secure
employees inside
the platform.
2.  Develop and enforce a workrule requiring employees to maintain firm
footing
on the floor of hte platform at all times and prohibiting employees from
climbing
on the guardrail to achieve additional reach or height.nal
