Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a high pressure water hose nozzle. a)Catfish Processing Area- On july 26, 2004 sanitation employees washed catfish processing equipment with high pressure blasting water hoses operating at a pressure of 1500 psi that were not provided with a fail safe valve/deadman control, exposing the sanitation employees to being struck by the metal nozzle if the employee lost his grip during cleaning operations. One feasible means to abate this hazard would be to follow standard industrial practice and install a fail safe valve/deadman (automatic shut-off) control on the high pressure water hose nozzle.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard when a sliding door automatically closes on them. (a) Employees are exposed to a crushing hazard while traveling through an automatic door that is not equipped with a stop/reverse safety device when the door contacts or detect an individual in its closing path. Among other methods, one feasible and acceptable method to correct this hazard is to install or retrofit the door with a sensor to detect persons while they travel through the door passageway and the sensor should cause the door to reverse direction so as not to injure the person.
