North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to
employees in that employees were exposed to fall hazards from a work
platform:
a)on December 23, 2003, an employee was working from a work platform that
exceeded the
maximum width allowable for platforms and that was not secured to the Lull
Rough Terrain
Forklift Truck.  The platform was observed to be ten feet above the lower
level, exposing
employee to a ten foot fall hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to
comply with the American National Standards Institute/American Society of
Mechanical Engineers
(ANSI/ASTM) B56.6 Safety Standard For Rough Terrain Forklift Trucks, which
limits the maxium
width of the platform to be the width of the load bearing wheels plus 10
inches and requires the
platform to be securely attached to the lifting carriage or forks.ge

employees in that employees were exposed to fall hazards from a work
platform:
a)on December 23, 2003, an employee was working from a work platform that
was being lifted
by a Lull Rough Terrain Forklift Truck.  While elevated, the employee used
the railings of the
platform to climb out of the work platform onto the roof.   The base of
the platform was
observed to be ten feet above the lower level and the roof was observed to
be 14 feet above
the ground, exposing employee to a fourteen foot fall hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to comply with the American National Standards Institute/American Society
of Mechanical
Engineers (ANSI/ASTM) B56.6 Safety Standard For Rough Terrain Forklift
Trucks, which
requires personnel to remain on the platform floor.rm
