were causing or likely to cause death or serious physical harm to

were exposed to:
(a) Tool Room Department, North Wall - The Rollin vertical band saw was
not equipped with
a magnetic restarter or similar device as prescribed by the American
National
Standard Institute
for Metal Cutting Saws, ANSI B11.10 - 1990, Section 5.2.3.4, that in event
an interruption in
the electrical supply, would prevent the saw from restarting automatically
when power is
restored.  Employees last used the vertical band saw on or about January
7, 20004.
(b) Maintenance Department, North Wall - The Grob vertical band saw was
not equipped with
a magnetic restarter or similar device as prescribed by the American
National Standard Institute
for Metal Cutting Saws, ANSI B11.10 - 1990, Section 5.2.3.4, that in event
an interruption in
the electrical supply, would prevent the saw from restarting automatically
when power is
restored.  Employees last used the vertical band saw on or about December
30, 20003.
