recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the rotating blade:
a)Maintenance Shop-On or about 1/08/04 the Kysor Johnson horizontal band
saw model B serial
#15395, was not equipped with a magnetic restarter or similar device, as
prescribed by
American National Standards Institute (ANSI) B11.10-1990, Section 5.2.3.4,
that in the event
of an interruption in the electrical supply such a device would prevent
the saw from restarting
automatically when power is restored.
b)Maintenance Shop-On or about 1/08/04 the Grob vertical band saw model
#NS18, was not
equiped with a magnetic restarter or similar device, as prescribed by the
American National
Standards Institute (ANSI) B11.10-1990, Section 5.2.3.4, that in the event
of
an interruption in
the electrical supply such a device would prevent the saw from restarting
when the power is
restored.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification. This documentation may include, but is not limited to,
evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
