Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the rotating blade: a)Maintenance Area-On or about 1/14/04 the Zipcut Friction saw no model#, was not equipped with a magnetic restarter or similar device, as prescribed by American National Standards Institute (ANSI) B11.10-1990, Section 5.2.3.4, that in the event of an interruption in the electrical supply such a device would prevent the saw from restarting automatically when power is restored. b)Maintenance Area-On or about 1/14/04 the Thomas Cold saw model#250, was not equiped with a magnetic restarter or similar device, as prescribed by the American National Standards Institute (ANSI) B11.10-1990, Section 5.2.3.4, that in the event of an interruption in the electrical supply such a device would prevent the saw from restarting when the power is restored. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)Main Production Area-On or about 1/14/04 the Chicago 6ft, 90 ton press brake was not equipped with a guard to cover the treadle which actuates the press, as prescribed by the National Standards Institute (ANSI) B11.3-2002, Section 6.10.3, that when provided, foot treadles shall be protected to prevent from unintended actuation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), andin addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
