employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that:
a)  Plastic Components, Inc. - North Ridgeville, Ohio:  On or about
January 16, 2004 the
employer failed to ensure that employees were protected from thermal burns
due to contact with
purged molten plastic from a horizontal injection molding machine (Press
#13 - Van Dorn -
serial number CV-500).  The purge guard was kept open by a bungee cord
while the machine
operated.  The injection mold machine was located on the molding floor.
b)  Plastic Components, Inc. - North Ridgeville, Ohio:  On or about
January 16, 2004 the
employer failed to ensure that employees were protected from thermal burns
due to contact with
purged molten plastic from a horizontal injection molding machine (Press
#24 - Nissei - serial
number S36E020).  The purge guard was open while the machine continued to
operate.  The
machine was located on the molding floor.
c)  Plastic Components, Inc. - North Ridgeville, Ohio:  On or about
January 16, 2004 the
employer failed to ensure that employees were protected from thermal burns
due to contact with
purged molten plastic from a horizontal injection molding machine (Press
#4 - Nissei - serial
number S26E073).  The purge guard was open while the machine continued to
operate.  The
machine was located on the molding floor.
d)  Plastic Components, Inc. - North Ridgeville, Ohio:  On or about
January 28, 2004 the
employer failed to ensure that employees were protected from thermal burns
due to contact with
an uncovered heated injecting barrel on a horizontal injection molding
machine (Press #1 -
Nissei - serial number S26E007).  The barrel guard had been removed from
the machine while
it continued to operate.  The machine was located on the molding floor.
One feasible means of abating this hazard is to install purge area guards
and injection barrel
guards, as supplied by the manufacturer of the specific injection molding
machines, and in
compliance with ANSI B151.1-1997, Sections 5.3.5 and 5.3.6 (Horizontal
Injection Molding
Machines - Safety Requirements for Manufacture, Care, and Use).

industrial truck (Nissan -
model CUB01-001081; serial number CUB01L155) was secured to the lifting
carriage or forks
of the industrial truck.  An employee was elevated to approximately 12
feet above the floor
while changing fuses in overhead electrical installations on the molding
floor.
One feasible means of abating this hazard is to secure the work platform
to the forks or mast
of the forklift in accordance with The Safety Standard for Low Lift and
High Lift Trucks -
ASME B56.1-2000, Sections 4.17.2, 4.17.3, and 7.35.3.
