The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees, in that:
a.Hale Chrome Service, Inc. located in Toledo, Ohio: On or about
February 8, 2005, the employer did not ensure the pendant
control buttons for the two ABUS 1000kg monorail chain hoists
located in the nickel plating department were marked as to the
identification of their functions.
b.Hale Chrome Service, Inc. located in Toledo, Ohio: On or about
February 8, 2005, the employer did not ensure the pendent
control buttons for the five ABUS 2000kg monorail chain hoists
located in the chrome plating department were marked as to the
identification of their functions.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to:
Install and maintain the pendent controls in accordance to ANSI
B30.11-1998, "Monorails and Underhung Hoists".
