Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that steel bolts creating an
impalement hazard were left protruding
upwards:
29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1
m) above a lower level shall be
protected from falling to that lower level:
While moving between upper and lower platforms on a tube and coupler
scaffold, subcontractor employees
climbed over guardrail systems, meant to protect them from falling.
Employees were exposed to an approximately
60 foot fall hazard.
Among other methods, feasible and acceptable abatement means to correct
this hazard includes, but is not limited
to:
Supplying access from level to level by means of ladders manufactured with
the
scaffold for use of access.
Completing the erection of the scaffold so that it would classify as a
large area scaffold and thereby qualify as a
scaffold on which a portable ladder can be used.

OSHA.
b)  The employer is responsible for selecting and implementing an
effective abatement method.
Abatement certification and abatement documentation is required for this
violation.  The documentation should
include written verification of abatement, applicable measurements or
monitoring results, and photographs or
videos which you believe will be helpful.  The abatement certification
sheet is enclosed with the citation(s).
