On or about 04/13/04, near the receiving area, employees were exposed to
the hazard of tip over
and being crushed by powered industrial trucks while operating forklifts
without wearing
seatbelts.
Among other methods of abatement, a feasible and acceptable method to
correct this hazard is
to prohibit employees from operating this type of equipment without
wearing seatbelts.

REQUIRED FOR THIS VIOLATION.
