Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: Powder Department; The employer did not ensure that the Yale 1/2 Ton and the P&H ZipLift 2000 lb. hoists, and related equipment, components and safety devices including but not limited to chains, ropes, hooks and under the hook lifting devices were maintained, inspected and tested in accordance with the intervals and requirements of ANSI/ASME B30.16, and ANSI/ASME B30.11 as applicable. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: to establish a program of inspections, testing, and preventative maintenance as recommended by the manufacturers and in accordance with the American National Standards Institute/American Society of Mechanical Engineers ANSI/ASME B30.11, Monorail Systems and Underhung Cranes - 1999 and ANSI/ASME B30.16, Overhead Hoists - 2001. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
