The freight elevator used by employees at the workplace had not been
maintained or inspected
within the last six months. Employees working around or useing the
elevator were exposed to
struck by or fall hazards.
Feasible and acceptable methods among other methods to corect the hazards
include, but are not
limited to, following the recommendations in accordance with the American
Society of
Mechanical Engineers ASME A17.1 1996.
1) Inspect the freight elevator every six months
2) Install  a self closing or swinging gate
3) Install a data plate that indicats the Code and edition in effect at
the time of installation.
29 CFR 1903.19(c)(1) requires certification that the abatement of the
above violation is
completed.
