Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by, crushed by hazards: Employees working in the following locations were exposed hazards associated with struck by, crushed by: a)Failure of lifting devices developed and built in house and used to lift including, but not limited to: welded bus floor frames, rolls of fabric padding, pre-formed fiberglass bus bodies. b)Failure of automobile lifting devices including, but not limited to: Challenger Lifts Model's #37000 and 39000, Herkules K-900 Series. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: a)Comply with ANSI B30.20 "Below the Hook Lifting Devices"; 1. Provide rated load on any custom-made lifting device. 2. Provide initial, frequent and periodic inspection of the lifting devices. 3. Ensure that initial proof testing of 125of the rated load was conducted. b)Implement a thorough scheduled preventative maintenance program as well as pre-lift inspections and tests as prescribed by the ANSI/ALI ALOIM-2000. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
