that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
(a) On or about March 18, 2004 at the establishment located at 701 Space
Center
Drive in West Memphis Arkansas an employee was exposed to a fall from
elevation
hazard of approximately 9 feet, while riding the forks of a forklift to
gain access to a
storage balcony. Abatement notes: Among others, one suitable method for
abating this
hazardous condition is: (1) Utilize only platforms designed for use in
elevating
personnel with forklifts. These platforms must meet the requirement set
forth in
ANSI/ASME B56.1-1993. (2) Re-train all employees about the hazard of falls
associated with improper elevating methods, and the proper application and
use of
safety platforms and personnel fall protection.

employees in that
employees were exposed to:
(a) On or about March 18, 2004 at the establishment located at 701 Space
Center
Drive in West Memphis Arkansas employees operating forklift trucks were not
required to wear seat belts when provided exposing employees to crushing
injuries
from overturning forklifts.  Due to the fact that the operator restraint
systems
(seat
belts) provided on the forklift trucks were not required to be used, as
required by
ASME B56.1-1993.
Abatement Note: One feasible and acceptable abatement method to correct
this
hazard is to require and effectively enforce the use of operator restraint
systems (seat
belts) while operating forklift trucks provided with them.
