Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish to each of his employees
employment and a place of employment which were free from recognized
hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to
hazards while riding on equipment.
(a) Exterior walls, AutoZone, 4079 Marietta Hwy, Dallas, GA, On or about
11/18/04 - Employees were exposed to
falls to a lower level from heights of approximately 12 feet, while riding
the forks of a Sunbelt SS-636 Turbo forklift
to access and exit scaffolding.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure (1) Utilize only platforms
designed for use in elevating personnel with forklifts. These platforms
must meet the requirement set forth in
ANSI/ASME B56.1-2000. (2) Re-train all employees about the hazard of falls
associated with improper elevating
methods, and the proper application and use of safety platforms and
personnel fall protection.
