Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of
1970: The employer did not furnish employment and a place of employment which we
re free from recognized hazards that were causing or likely to cause death or se
rious physical harm to employees, in that employees were exposed to being struck
by falling loads and/or falling hoist parts: The following deficiencies were no
ted on hoists and underslung cranes in the facility: (a)Overhead hoists (underhu
ng) did not have control buttons marked to indicate the direction of resultant m
otion that they controlled: (1) On or about April 27, 2005, the overhead hoist (
underhung) at the preheat conveyor did not have the four (4) buttons on the pend
ant control marked. (2) On or about May 4, 2005, the overhead hoist (underhung)
at the paint area drying conveyor did not have the control buttons on the pendan
t control marked.
(3) On or about May 5, 2005, the overhead hoist (underhung) at the Wheelabrator
did not have the control buttons on the pendant control marked. (4) On or about
May 6, 2005, Finishing area, the 4 button pendant control for the overhead under
slung hoist was not marked. (b)Safety latches on underhung hoists were damaged:
(1) On or about May 5, 2005, at booth #8 of the finishing area, the safety latch
on the hoist had been damaged and pushed out of the hook throat and was floppin
g around on the hook. (c)Overhead hoists (underhung) did not have stops or bumpe
rs installed on the rails to prevent hoists from being run off the rails: (1) On
or about May 4, 2005, QC/Old welding shop, no stops or bumpers were installed o
n the north end of the rail of the hoist used to move heavy castings.
(2) On or about May 5, 2005, Finishing Area Bridge Crane (underhung) #34, no sto
ps or bumpers were installed at the end the rail on crane used to move heavy cas
tings. (d)Load limits were not posted on overhead hoists (underhung): (1)On or a
bout May 4, 2005, Turret Room, load limit was not posted on #97 Overhead Hoist (
Underhung). (e)Wire rope load lines on overhead hoists (underhung) were damaged
with broken wires present: (1) On or about May 4, 2005, Q/C area, hoist had seve
rely damaged wire rope that was bent with broken wires sticking out. The rope wa
s not properly reeved on the drum. (2) On or about May 4, 2005, north finishing
area, hoist with 2000 pound lower block had wire rope which was bent with numero
us broken wires.
(3) On or about May 4, 2005, Paint area, hoist had wire rope load line with seve
ral damaged wires. (4) On or about May 5, 2005, Finishing area, hoist #34 had a
load line with numerous broken wires. (5) On or about May 5, 2005 Ladle Repair a
rea, hoist had a load line was severely damaged and covered by wheel grease. Amo
ng other methods of abatement, one feasible and acceptable abatement method to c
orrect the hazard is to develop a preventative maintenance program which incorpo
rates all manufacturers recommendations with written signed and dated reports an
d is also in accordance with ANSI B30.11 and ANSI 30.16 in order to ensure a saf

d acceptable abatement method to correct this hazard is to mark each push button

unexpected restart in electrical powered equipment after an electrical power fai
lure: (a) On or about April 28, 2005, the Rush drill sharpener, model #252, seri
al # 238, located in the tool crib, did not have a magnetic anti-restart switch
or other safety feature to prevent the machine from restarting. (b) On or about
May 4, 2005, Finishing room, small casting grinding area, the Double MM Drill pr
ess and the Goldens drill press, serial #0600007, did not have a magnetic anti-r
estart switch or other safety feature to prevent the machine from restarting. Am
ong other methods, one feasible and acceptable abatement method to correct this
hazard is to install after market anti-restart switches in the power supply.

rious physical harm to employees, in that employees were exposed to the hazards
of burns from unprotected heated surfaces and exposed flames as well as burns fr
om fire and explosion from unapproved space heaters: (a)On or about May 4, 2005,
QC area/old welding shop, the 'salamanda' type heater sitting on the floor next
to the south wall was not protected to prevent exposure to the hot metal shell
of the heater and from the potential dangers of damage to the gas regulator that
was located on the very bottom of the heater where it was being stepped on and
walked on by the employees.
(b)On or about May 18, 2005, the field manufactured heater used to heat the flux
container at the Spin Cast furnace had unprotected heated surfaces, flames prot
ruding from the gas heating device, around the container and was not provided wi
th a safety valve which would turn the gas off in the event of a flame failure.
(c)On or about May 18, 20205, the unapproved, field manufactured heater on the f
loor of the spin cast area was a five gallon bucket, that was turned upside down
over a gas flame, that was not provided through an approved safety valve which
would turn the gas off in the event of a flame failure. Among other methods, one
feasible and acceptable abatement method to correct this hazard is to remove th
e unapproved heaters from service and replace them with UL approved heaters. ABA

m a ladder: (a) On or about May 18, 2005, the 20 foot upper section of the fiber
glass extension ladder located just east of the spin cast area had been separat
ed from the bottom section and used as separate ladder for access up to the elev
ated sections of the equipment. The separation and use of the extension ladder s
ections was a use contrary to the way the ladder was approved for use. ABATEMENT
DOCUMENTATION AND CERTIFICATION REQUIRED Among other methods, one feasible and
acceptable abatement method to correct this hazard is to comply with Section 9.3
.10 of the American National Standards Institute (ANSI) A14.5-1992, Safety Requi
rements for Portable Reinforced Plastic Ladders and ensure that top sections of
sectional
ladders are not used as a separate ladder unless the user equips them with safet
y feet.y
