Title 40.1-51.1.(a), Code of Virginia:  The employer did not furnish
employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in
that:
There was no fixed vertical ladder installed in the 53 1/4" deep pit for
safe access as required in pits over 36"
deep by ANSI A17.1-1978, Section 106.1d.2, American National Standard for
Elevators, Dumbwaiters, and
Moving Walks, a nationally recognized consensus document.
One feasible and acceptable abatement method, among others, to correct
this hazard is to install the proper
access ladder.
(b)The ground-level access door to the elevator pit was not of a
self-closing type and was not provided with a
spring-type lock arranged to permit the door to be opened from the inside
of the pit without a key as required
by ANSI A17.1-1978, Section 106.1d.3, American National Standard for
Elevators,
Dumbwaiters, and
Moving Walks, a nationally recognized consensus document.  This resulted
in an employee of the building
occupant opening the access door from the outside and falling into the pit
and receiving serious injuries.
"One feasible and acceptable abatement method, among others, to correct
this hazard is to install the proper
self-closing fire door with a "panic bar" type latching device.g
