General-duty citation text
Section 5(a)(1) of the Occupational safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. On or about February 2, 2004, at the above addressed jobsite, the employer did not install the Pecco PH6000-12D/Tornborgs Magni HS-3827 personnel & material hoist according to the manufacturers specifications as documented in drawing number 23502-4 Alternate Anchor Bolt Detail in that: 1. The base section was not properly bolted to the concrete slab. 2. The hoist car did not remain at ground level until the first mast brace was installed. Employees were thereby exposed to the hazards associated with a falling hoist.
