5(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees, working in the Tool and Die Repair area, were exposed to crushing and amputation hazards caused by a press die slipping and falling from an Armstrong heavy duty pattern C-Clamp used for lifting and hoisting operations. Feasible and acceptable abatement methods include drilling and tapping the die to accept the use of threaded lifting points; or using a Below-The-Hook Lifting Device, that meets the requirements outlined in ASME B30.20-1999, to properly engage and support the die during lifting operations.
