P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of being struck by the overhead hoist or structural components due to failure of the beams not capable of supporting the load: a) Welding shop - Employees were exceeding the posted load rating on the beams of the overhead electrical chain hoist. Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace existing structural components with components that will support the intended load and re-train employees to ensure that the posted load rating is not exceeded. For additional assistance, please refer to ANSI B30.16-1973 Overhead Hoists, Chapter 16.1, Section 16.1.3.2.1.

P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falling from ladders positioned on scaffold platforms: a) Welding shop - An employee was working from a 6 foot portable aluminum step ladder that was placed on the platform of a mobile scaffold, exposing him to fall hazards. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that employees use scaffolds of sufficient height to reach the work area. For additional assistance, please refer to ANSI A14.2-1972 Safety requirements for portable metal ladders, Appendix A3, Section A3.8.
