SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  Employer knew or should have known that no back-up alarm on Bobcat 863
skid-steer machine, when operated in reverse gear, at side lot area, last
occurring about one week prior to OSHA inspection, constituted a hazard of
being backed over.  A feasible and useful method, among others, to correct
this hazard is to equip machine with an operable back-up alarm.
B.  Employer knew or should have known that no seat belt worn by operator
of
Lull 1044C-54 forklift, at side lot area, constituted a hazard a being
thrown
from the forklift.  A feasible and useful method, among others, to correct
this hazard is to wear the seat belts in the forklift.
