SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees in platform elevated by forks of Lull 644B-42 forklift, old Rental Number FLH-1103, at front area of building were exposed to hazards such as falling due to platform slipping from the forks. A feasible and useful method, among others, to correct this hazard is to firmly secure the platform to the forks or lifting carriage and have the operator remain at controls when employee is elevated.
