employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,

A.  Employer knew or should have known that employees using the Toyota
Deka #2
forklift were exposed to the hazard of being thrown from the forklift and
crushed due to no seat belts being provided.  A useful and feasible means
of
abatement, among others, would be to install a seat belt on the Toyota
forklift located in the warehouse department of establishment.
