employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,

A.  Employer knew or should have known that employee operating forklift was
exposed to the hazard of falling from or being pinned or crushed underneath
forklift.  A feasible and useful method to correct this hazard, among other
methods, is to require seat belt to be worn on Yale forklift, #096, Serial
Number E108V145754, at plant.
