Mitsubishi forklift, Serial Number AF91A-52072, was exposed to the hazard
of
being thrown from the driver's seat and crushed.  A feasible and useful
means
of abatement, among others, is to install and require the use of a seat
belt
in Warehouse #1 (Diebold Warehouse) of the establishment.
