Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:06/19/04 Equipment:John Deere Heavy Duty Flex-Wing Rotary Cutter, Model #HX 15, Serial #W00HX15E004926, John Deere Tractor, Model #5420, Serial #LV5420T445074 Condition:An employee, without specific equipment training or sufficient reference material, connected rotary cutter hydraulic lines in the incorrect tractor selective control valve couplings which resulted in insufficient hydraulic support to keep the cutter wings from falling. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide specific equipment training and all necessary service reference material to perform maintenance on rotary cutters and tractors. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
