On 5/6/04, three employees were riding in the bed of a pickup truck.  Two

were sitting on the tailgate and fell off after the tailgate cables
snapped.  One employee
incurred a fatal injury.

correct this hazard is
to provide employees with an appproved seat and seat restraint system.
