furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by and/or crushed by hazards of the horizontal
earth boring machine:
(a)Adjacent to 2340 John Hawkins Pkwy, Hoover, Alabama - American Augers
model 36-600S Split Turbo Boring Machine - On or about 04/01/2004, the
hook-
rollers were not secured to the track.
Among other methods, one feasible and acceptable method to correct this
hazard
is to ensure the machine is secured to the track as required by the
manufacturer.
