On or about January 27, 2005, at least two employees were exposed to the
hazards of falling
approximately 15-feet when using a personnel basket lifted by a
Caterpillar TC60D forklift as
a means to descend from the work area of the roof.
Among other feasible methods to control the hazard:
1.  Use an extension ladder to access the roof.
2.  Comply with ANSI B56.1-1969 paragraph 603(L), Operating Safety Rules
and Practices;
Lifting personnel.
3.  ASME B56.6-1992 paragraph 8.25 safety standards; Platforms for lifting
personnel.
