were causing or likely to cause death or serious physical harm to

On the north side of the newly constructed ambulance entrance.
On or about March 25, 2005 at least one employee, who rode a pallet that
was placed on the
forks of a forklift to access the roof, was exposed to a fall hazard.
Among other methods, one feasible abatement method includes:
1.  Using an extension ladder to access the roof.
2.  Comply with ANSI B56.1-1969 paragraph 603(L), Operating Safety Rules
and Practices;
Lifting personnel, and
3.  ASME B56.6-1992 paragraph 8.25 safety standards;  Platforms for
lifting
personnel.
