On or about 12/13/2005, the employer did not ensure that employees
operating forklift were
utilizing personal restraining devices such as seat belts. Forklifts are
operated throughout the
warehouse and in the packaging department.
Among other possible methods, one feasible means of abatement for this
hazard is the use of
seat belts as outlined in ASME/ANSI B56.1 standard for powered industrial
trucks. Older
forklifts may have retrofit
