Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)F/V Bering Prowler, Factory Deck: On or about February 2, 2008 and at times prior thereto, energy isolation procedures (lockout/tagout, LO/TO) procedures had not been initiated when performing blade removal on the Tokai (J Cut) Header machine. The employee was exposed to severe laceration and possible amputation hazards which would require hospitalization. Among other methods, one feasible and acceptable abatement method to correct the hazard is to follow the comprehensive lockout/tagout program the employer had developed. Abatement verification and documentation required.
