causing or likely to cause death or serious physical harm to employees in

exposed to struck-by hazards:
(a)Harbor Crown Marine Railway, T/V Saratoga:  On or about June 18, 2008
and at times
prior thereto, whip protection was not provided on the 1 and 1/4 inch
compressed air
hose originating from the Ingersoll Rand model number 185, Equipment
number AC17
air compressor and which terminated at the unfired pressure vessel.
Output of the
compressor indicated 210 p.s.i.g.  Employees were exposed to struck-by
hazards in the
event the hose released from the termination.
(b)Harbor Crown Marine Railway, T/V Saratoga:  On or about June 18, 2008
and at times
prior thereto, whip protection was not provided on the 2 inch compressed
air
hose
originating from the unfired pressure vessel and terminating at the Clemco
Industries
Blast Pot requiring an operating pressure of 90 to 160 p.s.i.  Employees
were exposed
to struck-by hazards in the event the hose released from the origination.
Among other methods, one feasible and acceptable abatement method to
correct the hazards
would be to install hose-coupling safety locks and hose whip checks.
