(a)Shelford's Boat LTD (F/V Aleutian Lady) Passageway and Engineering: On
or about
February 1, 2009, unqualified employees were exposed to electrical shock
hazards due
to unguarded openings exposing energized busbars or controller parts and
inadequate
labeling on:
1. Passage panel: 208V (3) openings 1" x 2 1/4" each. Labeling
inadequate,
2. Shop panel: duct tape over breaker opening. Inadequate
labeling,
3. Engine Room: Lower compressor motor junction 208V (no
covers on box),
4. Engine Room: Port generator area 208V 300 Amp breaker
multiple openings,
5. Engine Room: Harbor generator panels tied into port MCC
panel 4" x 3" opening (1) on inboard and (6) Openings on
outboard panel.
Among other methods, a feasible means of abatement would include
protecting open circuits
with a cover as required by the 2008 NEC article 110.27(A)(2) and legibly
labeling disconnects
within panels as required by the 2008 NEC 110.22(A).
