29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  (a)  Engine Space:  On or about February 10, 2016 and at times prior thereto, where sound level measurements of 97-105 decibels were measured, the employer had not developed or implemented a monitoring program.  This condition exposed employees to hearing loss.  (b)  Refrigeration Space:  On or about February 10, 2016 and at times prior thereto, where sound level measurements of 85-106 decibels were measured, the employer had not developed or implemented a monitoring program.  This condition exposed employees to hearing loss.  (c)  Gear Alley:  On or about February 10, 2016 and at times prior thereto, where sound level measurements of 92 decibels were measured, the employer had not developed or implemented a monitoring program.  This condition exposed employees to hearing loss.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  Engineering Space:  On or about February 10, 2016 and at times prior thereto, three (3) 3M, half-face, negative pressure respirators were stored in a manner in which the facepieces were subject to contamination.  This condition exposed employee to inhalation hazards when donning the contaminated equipment.
