(a)VT Griffin (Waste Water Treatment Plant): On or about July 29, 2009,
employees were
exposed to a 6' fall hazard off a Werner 6' step ladder used as an
extension ladder to
access the fixed ladder on the side of the sludge truck when inspecting
the level in the
tank.
One feasible method of abatement could be to install a hinged and movable
ladder extension to
the bottom of the truck ladder which could be used to access the higher
fixed ladder on the side
of the truck.
