Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
(a)Zenith Alaska LLC (F/V Zenith): On or about February 8, 2010, employees
were
exposed to an electrical shock hazard caused by an unused 2 1/2" x 3/4"
opening in the
energized 120V panel located in the galley which was accessible to all
untrained
employees on the vessel.
(b)Zenith Alaska LLC (F/V Zenith): On or about February 8, 2010, an
employee of the
vessel and a contractor employee were exposed to electrical shock hazards
caused by:
1.The employer not developing an energy control program which was
inclusive of all
required elements and employee training, although employees performed
shipboard
repairs and service on mechanical equipment and machinery.
2.The employer not periodically evaluating the LO/TO procedures required
by the
standard.
3.The employer not providing training for employees to ensure their
understanding of the
purpose and function of the LO/TO program and to verify the knowledge and
skills
required.
4.The employer not verifying that the energy isolation had been
accomplished through a
proper LO/TO procedure due to the vessel-provided tag not completed and the
improperly installed device falling off of the breaker controlling the
480V main hydraulic
pump being worked on in the engineroom.
5.The employer nor ensuring that the vessel LO/TO procedures were reviewed
properly
with the LO/TO procedure of the contracting employer who was repairing the
main
hydraulic pump on the vessel.
One feasible means of abatement among other methods would be to:
(a) Inspect and repair all damage to electrical panels and devices on a
regular basis.
(b) Expand on the current LO/TO Program to include all required elements
and provide training
for all authorized and contractor personnel working on the vessel required
to perform LO/TO
or work on equipment and machinery required to be locked out.
