Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)Fire Control Systems Inc. (Chevron Anna Platform): On or about March 30, 2010, an employee was exposed to a struck-by hazard caused by an employee using a 12" Snap-On wrench to pry off the cap of an Ansul portable dry-powder fire extinguisher. This action went against the manufacturer's requirements, since it was possible to create fatigue cracking in the threads and cap leading to weakening of the cap under the 200 psi pressure exerted by the gas cylinder in the event the extinguisher is used. One feasible method of abatement might include using only the nozzle handle to gain leverage for turning the cap and to not authorize any other form of prying tool to be used when performing the task.
