29 CFR 1910.119(d)(3)(ii):  The employer did not document that the
equipment in the process
complied with recognized and generally accepted good engineering practices:
(a)P/V Arctic Star:  On or about July 8, 2010, and at times prior thereto,
direction of flow
and refrigerant state labeling had been removed and not replaced in areas
including but
not limited to the high pressure receiver deck, low pressure receiver and
cold storage,
and condenser room.  Piping identification was not being maintained as
required by the
Process Safety Management (PSM) Program which incorporated by reference
ASHRAE
15, ASHRAE 26, ANSI A13.1, and IIAR Bulletin 114.  This condition exposed
employees to inhalation and eye and skin burn hazards in the event of an
uncontrolled
release of anhydrous ammonia.
(b)P/V Arctic Star:  On or about July 8, 2010, and at times prior thereto,
valve
identification tags were either missing, removed, taped over, or painted
over on
equipment including but not limited to the high pressure receiver, Sullair
Room oil
separator, compressors including high stage, swing, boosters, condenser
room, and air
purger system.   Valve identification was not being maintained as required
by the PSM
Program which incorporated by reference ASHRAE 15 and ASHRAE 26.  This
condition
exposed employees to inhalation and eye and skin burn hazards in the event
of an
uncontrolled release of anhydrous ammonia.
ICICLE SEAFOODS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS
OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN
OSHA INSPECTION NUMBER 307504050, CITATION NUMBER 1, ITEM NUMBER 2A,
ISSUED ON 04/22/2009, WITH RESPECT TO A WORKPLACE LOCATED AT P/V BERING
STAR, DUTCH HARBOR, ALASKA.

29 CFR 1910.119(j)(4)(ii):  Inspections and testing procedures performed
on process equipment
to maintain its mechanical integrity, did not follow recognized and
generally accepted good
engineering practices:
(a)P/V Arctic Star:  On or about July 8, 2010, and at times prior thereto,
the inspection
frequency for temperature and pressure gauges for process equipment
including but not
limited to compressors, pressure vessels, condensers, and oil separators
had not been
completed.  The Process Safety Management (PSM) program incorporated IIAR
Bulletin
110 as an acceptable engineering practice.  This condition exposed
employees to
inhalation and eye and skin burn hazards in the event of an uncontrolled
anhydrous
ammonia release onboard the vessel.
(b)P/V Arctic Star:  On or about July 8, 2010, and at times prior thereto,
the calibration
frequency for portable and fixed anhydrous ammonia monitoring equipment
was not
maintained.  The Process Safety Management (PSM) program incorporated IIAR
Bulletin
110 as an acceptable engineering practices.  This condition exposed
employees to
inhalation and eye and skin burn hazards in the event of an uncontrolled
anhydrous
ammonia release onboard the vessel.
(1)Manning Systems Model# EC-P1-NH3, Serial#'s 0321 and 0709, which
were in use and due calibration "07/08."
(2)International Sensor Technology Model# SM95, Serial# SM4785-9939 was
in use and showed a last alarm inspection of "2-20-10."
ICICLE SEAFOODS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS
OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN
