Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: Two employees were exposed to fall hazards approximately 10 feet while working from a personnel basket attached to the fores of a rough terrain forklift that was telescoped to the second floor. The employees were not wearing any personal fall arrest equipment. Location: 1900 Premier Dr. Country Inn & Suites, Mankato, MN

Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: The personnel basket attached to the Lull forklift was approximately 16 feet in length with a wheelbase of approximately 8 feet wide. This exposed two employees to fall hazards due to tipping the rough terrain forklift. Location: 1900 Premier Dr. Country Inn & Suites, Mankato, MN
