Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employmnent and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fall hazards and
entanglement hazards:
(a) SoundTrack Audio & Video, at 1370 S. Colorado Blvd., Denver, CO 80222:
On or before March 16, 2004 the employer allowed employees to ride the
"upbelt"
conveyor. Riding the conveyor exposed three or more employees to the
hazard of
falling from the belt or to the hazard of becoming entangled in the belt.
(b) On or before March 16, 2004 the employer allowed employees to ride the
"downbelt" conveyor. Riding exposed three or more employees to the hazard
of
falling from the belt or to the hazard of becoming entangled in the belt.
One feasible method of abatement, among other methods, is to prohibit
employees from
riding the conveyor belts in accordance with industry consensus standard
ASME B20.1-2000
5.12d, which states that, "No person shall ride on a conveyor, except on a
slow moving
assembly conveyor 0.4m/s (80 ft/min) maximum or on a conveyor that
incorporates a station
specifically designed for operating personnel."
Abatement Note:  Abatement certification is not required for this item

On or before March 16, 2004 the employer did not ensure that there were
emergency
pullcords, or other similarly effective devices, on the sides of the
"upbelt" conveyor.
Riding the conveyor belt and working in close proximity to the belt without
emergency stop devices exposed three or more employees to the hazard of
entanglement.
(b) On or before March 16, 2004 the employer did not ensure that there were
emergency pullcords, or other similarly effective devices, on the sides of
the
"downbelt" conveyor. Riding the conveyor belt and working in close
proximity to the
belt without emergency stop devices exposed three or more employees to the
hazard
of entanglement.
One feasible method of abatement, among other methods, is to install
emergency stop devices
in accordance with ASME B20.1-2000 5.11.2c, which states that, "Remotely
and
automatically controlled conveyors, and conveyors where the operator
stations are not
manned or are beyond voice or visual contact from drive areas, loading
areas, transfer
points, and other potentially hazardous locations on the conveyor path not
guarded by
location, position, or guards, shall be furnished with emergency stop
buttons, pull cords,
limit switches, or similar emergency stop devices."

audible
and visible warning alarm indicating that movement of the "upbelt"
conveyor was
initiating.  Employees riding the conveyor and those working in close
proximity
to the
conveyor were exposed to fall hazards and entanglement hazards as they
cleared
materials jamming the conveyor.
(b) On or before March 16, 2004 the employer did not ensure that there was
an
audible and visible warning alarm indicating that movement of the
"downbelt"
conveyor was initiating.  Employees riding the conveyor and those working
in close
proximity to the conveyor were exposed to fall hazards and entanglement
hazards as
they cleared materials jamming the conveyor.
One feasible method of abatement, among other methods, is to install horns
and strobe lights,
which are linked to the conveyor activation switch, and which allow for a
delayed response.
These warning devices shall be installed in accordance with industry
consensus standard
ASME B20.1-2000 5.11.2(b)(1)(d).
