Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of amputation: a) Stainless Fabrication Company, 860 Navajo Street, Denver, CO: Located on the 160 ton Aizawa hydraulic press brake machine, the return control button was non-functional. Among other methods, one feasible and acceptable abatement methods to correct this hazard would be to follow the American National Standard consensus standard, B11.3-2002 section 6.1.4, and ensure that all hydraulic press brake machines equipped with a "stop return" control buttons are properly working. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
