Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed arc flash and arc blast:
(a)Kestrel Management L.P. at 1999 Broadway Blvd., Denver, CO 80201: On
March 1, 2005 two employees were exposed to electrical hazards when
opening electrical panels without performing a Flash Hazard Analysis.
Employees used a procedure to manually unlock the latch from the right side
of the cabinet with a screw driver and move the latch on the top front to
allow
the panel to open.  This procedure conducted without performing a Flash
Hazard Analysis exposed the employees to arc flash and arc blast when they
approached a bus from approximately 15 inches that was energized to 480
Volts phase to phase.

Flash Hazard
Analysis describes the flash hazard analysis process and equipment to
protect
personnel from
the possibility of being injured by arc flash.  The analysis shall
determine the flash protection
boundry and the personal protective equipment that employees within the
flash protection
boundry shall use.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to follow established procedures as described in NFPA 70E as well as
ensure a detailed
inspection of the cabinets is performed when the infrared scans are
conducted.
(b)Kestrel Management L.P. at 1999 Broadway Blvd., Denver, CO 80201: On
March 1, 2005 two employees were exposed to electrical hazards when
opening electrical panels in that the panels were not properly marked to
warn
of potential electric arc flash hazards. This condition exposed the
employees to
arc flash when they approached a bus energized to 480 Volts phase to phase
from approximately 15 inches.
Abatement Note: The National Electrical Code Handbook paragraph 110.16
gives direction
that switchboards, panelboards, industrial control planels, and motor
control centers shall be
field marked to warn qualified persons of potential electric arc flash
hazards.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to follow established procedures as described in the NEC paragraph
110.16.
