(a)Rocky Mountain Transload, Inc. & 1001 W. 52nd Ave., Denver, CO
80221: On or before March 30, 2005, an employee rode the bucket of the JCB
front-end loader from the ground up to the chute of conveyor # 2 to
dislodge
material stuck in the conveyor. This condition exposed the employee to
hazards of falls approximately 15 feet, to being struck by the bucket, to
being
caught in-between moving parts of the bucket and the conveyor.
Abatement Note: Abatement certification is required for this item (see
enclosed "Sample
J153, Surface Vehicle Recommended Practice, Section 4.3 Operator-General
Precautions,
instructs operators: "Do not permit riders on the machine if there are no
manufacturer's
designated place for the rider." In addition, Section 7.10, Operating,
Precautions, instructs
operator's to never allow anyone to be under or within reach while the
equipment is
operating.
