were causing or likely to cause death or serious physical harm to

were exposed to a struck by hazard while engaged in the placement of
concrete for a
basement slab through a concrete pump.  Employees were working adjacent to
the end
of the concrete pump hose while the pump was started and restarted.  This
is a violation
of the Concrete Pump Manufacturers Association procedures for safe
operation, Safety
Standard for Concrete Pumps, Placing Booms and Delivery Systems, Section
27-
1.18.2.2.
