Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a)22 Schiavonne Rd., New Windsor, NY - on or about June 7, 2005 - an
employee
was exposed to a struck by hazard while engaged in the placement of
concrete for a
basement slab while operating this pump adjacent to the delivery hose
during starting and
restarting of the pump.
Methods to correct this hazard include, but are not limited to:  1)
Develop and implement
a comprehensive safety training program for employees on the use of this
equipment.
2) Follow Reed Manufacturing's procedures in the section headed "CLEARING A
PACK OR BLOCKAGE" that outlines procedures for safely clearing a blocked
pump
line.  3) Follow the CPMA 27-1.18.2.2 paragraph# 10 procedures that state:
Personnel
should be cleared from the area of the end hose when starting or
restarting the
pump.
