hazards of being struck by
or crushed by falling equipment:
a)lift truck shop:  the hoist used to remove and reinstall engines and
transmissions
weighing approximately 300 pounds each did not have any record of any type
of
inspection performed on it to ensure proper operation.  [reference:  ASME
HST-2M-1989, paragraph 3.4].
b)lift truck shop:  the hook used with the hoist that is used to remove
and reinstall
engines and transmissions weighing approximately 300 pounds each did not
have a
throat latch installed where there was a place for the latch and the hook
was bent
laterally out of alignment.  [reference:  ANSI B30.16-1981, Section
16-2.1].
Among other means, one feasible and acceptable abatement method to correct
thess hazards
would be to institute the required inspections and to replace the hook.ce

personal fall arrest systems and
