North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: a)falling from the Wilkie Model 380 Ladder Truck - employees working on the ladder truck approximately 12 feet above the ground were not protected from falling by a means of personal fall protection such as a full body harness. NOTE:Reference Manufacture Guidelines - Order of Operations Procedure, Page 4 - O.S.H.A requires the use of a full body harness and lanyard when operating this equipment.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: a)falling from the Wilkie Model 380 Ladder Truck. Employees with a combined weight in excess of 350 lbs. were working from the boom of the ladder truck approximately 12 feet above the ground. Manufacture Capacity Ratings states doe not exceed capacity of 325 lbs. NOTE: Reference Order of Operations Procedure, page 5 - Capacity Ratings: Platform capacity 325 lbs. at a combined maximum extension and minimum degrees of elevation.
