Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards due to the absence of a functional hoistway
door
interlock and the
absence of adequate illumination in and around a freight elevator.  On or
about June 3, 2004,
there was inadequate lighting in and around elevator #95309, at the first
floor level, in Building
2 at 153 Hamlet Avenue.  Also, this elevator's hoistway door interlock,
which prevents the
hoistway doors from being able to open while the elevator car is not
there, was inoperable.  In
Building 1, elevator #95311 also did not have adequate illumination in and
around the elevator
car.  Illumination inside of these elevator cars was not in accordance
with Section 3.4.5 of
ASME A17.3-2002, Safety Code For Existing Elevators and Escalators.
Feasible and acceptable
abatement methods to correct this hazard include providing adequate
illumination outside of the
elevators as well as in the interior of the elevator cars in accordance
with ASME A17.3-2002.
Also, the hoistway door interlock on elevator #95309 shall be repaired and
shall be functional
in accordance with Section 2.7.1 of ASME A17.3-2002.

employees in that employees
were exposed to fire hazards due to accumulation of combustible trash and
debris in the pit of
an elevator.  On or about June 3, 2004, there was an accumulation of
combustible trash and
other debris at the bottom of the pit of elevator #95309 in Building 2 at
153 Hamlet Avenue.
One feasible and acceptable abatement method to correct this hazard would
be to have a licensed
elevator technician clean the pit area of this elevator and maintain it in
accordance with Section
8.6.4.7 of ASME A17.1-2004, Safety Code For Elevators and Escalators.
