were causing or likely to cause death or serious physical harm to

were exposed to the hazard of falls, crushing, struck by and amputations:
On or about February 22, 2005, employees operating the Economy Baler,
Serial
#54929, were exposed to the hazard of falling into the hopper, and
possibly being
crushed, due to the feed area being inadequately guarded.  Employees
loading
debris into the baler were also exposed to moving debris from the chute.
"AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE ABATEMENT
METHODS TO CORRECT THE HAZARDS ARE:
a.Eliminate access to open loading area;
b.Enclose chute or feed hopper in loading area;
c.Interlock on chamber door closure in accordance with ANSI
Z245.5-1990, Section 5.4;
d.Training of employees operating the baler in accordance with
ANSI Z245.5-1990, Section 7; and/or,
e.Implement inspections and maintenance of baler in accordance with
ANSI Z245.5-1990, Section 6.4-5.
